Primary health care plan. Family physicians experience in a community of the municipality of Tunja.

The family care plan allows to formulate a proposal for the health care of each individual belonging to a family, according to the findings and / or evidenced needs to promote and maintain health in an integral way. Objective: To show the family care plan, as a fundamental tool of family medicine for a comprehensive and integrative approach, of the families in the Patriot neighborhood of the city of Tunja. Methodology: A contextualization was carried out on the neighborhood of the patriots, in the city of Tunja; Basic concepts such as: family medicine, primary health care and the social determinants of health are considered useful in understanding the family care plans carried out by residents of the Family Medicine specialization at the Pedagogical and Technological University of Colombia (UPTC), in some families that live in the neighborhood of the patriots of the city of Tunja. Conclusions: The human being is by nature a social being, who is influenced by his developed environment within the functional unit of society, the family, which in turn is influenced by the social determinants of health; It is for this reason that they use the health care plans, it allows to have a holistic vision of the individual and his family.
